DOCUMENT OVERVIEW
Netherlands Health Declaration Form is a new travel screening required for all international travelers who wish to leave or enter the Netherlands. This form was created as a measure to prevent the spread of coronavirus among residents and non-resident travelers. It does not replace a visa.
WHO CAN ENTER THE NETHERLANDS WITH THIS DOCUMENT
All travelers aged 13 and over must fill out the Netherlands Health Declaration Form at least 24 hours before entering or exiting the country
All nationalities can apply.

Although a negative PCR COVID-19 test result is not required to fill out the application, certain travelers will still need to take a test to the country’s safety protocols. Do I need a COVID vaccine or vaccination certificate to enter Netherlands?
Yes, Netherlands has been taking steps to restore travel within its borders. If you are coming from a high-risk country or a very high-risk country, in order to enter the Netherlands, you’ll need to display your COVID-19 vaccination certificate.
Approved vaccines are Pfizer, AstraZeneca, Johnson & Johnson, Moderna, Serum Institute of India, Sinopharm BIBP, Sinovac.
You are considered vaccinated 2 weeks after receiving the second dose of a vaccine against COVID-19, or 28 days after receiving the Janssen vaccine.
Do I need to take a COVID-19 PCR test before traveling to the Netherlands?
- COVID test prior to arrival: PCR within 48 hours/antigen within 24 hours
The following travelers are exempt from pre-departure testing:
- Fully vaccinated/recovered travelers (apart from arrivals from very high-risk areas and outside the EU/Schengen Area)
- Travelers coming from Safe countries and areas participating in the EU travel rules scheme (check the country list FAQ to find out more)
- Children under 12

Do I have to quarantine when I get to the Netherlands?
- Quarantine requirements: 10 days for all travelers arriving from very high risk areas and countries with a virus variant of concern.
- Quarantine testing: Day 5 test to end quarantine early with negative result.
