DOCUMENT OVERVIEW
The Indonesia Health Alert Card is a health screening document implemented by the Indonesian authorities so that travelers can be easily contacted in case of any risk to their health. All visitors to Indonesia must complete this document to be allowed entry. The application will generate a QR code that must be presented on arrival. It does not replace a visa.
WHO CAN ENTER INDONESIA WITH THIS DOCUMENT
All travelers to Indonesia are required to complete this form. Foreign nationals who have been to Angola, Botswana, Eswatini, Hong Kong, Lesotho, Malawi, Mozambique, Namibia, South Africa, Zambia or Zimbabwe in the 14 days prior to their travel to Indonesia are not allowed to enter the country.
OBTAIN THE INDONESIA HEALTH ALERT CARD
To get this document, you’ll need to submit data about your health status, your upcoming trip to Indonesia, and personal information like your birth date and passport number. You must own a confirmed flight ticket to complete the form.
DISPLAY THE DOCUMENT
Once you submit your application, we’ll forward the finalized Indonesia Health Alert Card to your email before your trip. That way, you’ll be ready to display the document and the QR code when you get to the airport.
To enter the country, all travelers must provide proof of a negative COVID-19 PCR test result (in English) issued within 72 hours before departure to Indonesia, proof of full vaccination and complete a 5-day quarantine at designated facilities. In addition, all travelers must take two additional PCR tests: on day 1 and day 4 of quarantine.

The Indonesia Health Alert Card is part of the documentation you will need to enter Indonesia, it DOES NOT replace a visa. If your country requires a visa, the immigration officers will ask for your visa when you enter Indonesia. Keep in mind some nationalities will require a paper visa. If that is the case, we suggest contacting your local embassy.

Eligible travelers will be allowed to enter Indonesia with the following:

Eligible travelers are citizens from the following countries on tourist/limited stay visas: Saudi Arabia, United Arab Emirates, New Zealand, Kuwait, Bahrain, Qatar, China, India, Japan, South Korea, Liechtenstein, Italy, France, Portugal, Spain, Sweden, Poland, Hungary and Norway

Do I need a COVID vaccine or vaccination certificate to enter Indonesia?
Yes, Indonesia has been taking steps to restore travel within its borders. In order to enter Indonesia, you’ll need to display your COVID vaccine passport or vaccination certificate.
Accepted vaccines are Pfizer, Moderna, Sinovac, AstraZeneca, Johnson & Johnson, and Sinopharm.
Is a COVID-19 test required to enter Indonesia?
- COVID test prior to arrival: PCR (in English) within 72 hours.
- COVID test after arrival: PCR on day 1 and day 9 of quarantine.
Do I need to quarantine when I get to Indonesia?
PLEASE NOTE: Only fully vaccinated travelers from eligible countries can enter Indonesia
